The Next Generation ACO Model ended on December 31, 2021. As a result, Pioneer Valley Accountable Care has transitioned to the Medicare Shared Savings Program effective January 1, 2022.

PVAC is an Accountable Care Organization (ACO) located in western Massachusetts serving Medicare fee-for-service beneficiaries throughout the Pioneer Valley.
You may be wondering what an "ACO" is; Medicare ACOs are comprised of groups of doctors, hospitals, and other health care providers and suppliers who come together voluntarily to provide coordinated, high-quality care at lower costs to their Original Medicare patients, ensuring patients receive the right care, at the right time, in the right setting.
PVAC is participating in the Medicare Shared Savings Program, a population-based payment initiative for health care organizations and providers who are experienced in coordinating care for patients across care settings. Medicare created this Model to improve the quality of care for Medicare fee-for-service beneficiaries through care coordination and disease management. PVAC's goal is to work closely with your doctor to help meet your healthcare needs. A list of participating physician groups, and hospitals can be found on the "MSSP Public Reporting" link located on the navigation bar. The participating providers on the list have chosen to work with PVAC because they are committed to providing patients with a high quality health care experience. The goal of the MSSP is better health for populations, better care for individuals, and lower growth in expenditures.
Any patient who has multiple doctors probably understands the frustration of fragmented and disconnected care. ACOs are designed to improve the partnership between patients and doctors in making healthcare decisions. Medicare beneficiaries will have improved control over their health care, and their doctors can provide quality care because they will have improved information about their patient's medical history and can communicate with the patient's other doctors. Medicare beneficiaries whose doctors participate in an ACO will still have a full choice of providers and can still choose to see doctors outside of the ACO. Patients choosing to receive care from providers participating in ACOs will have access to information about how well their doctors, hospitals, or other caregivers are meeting quality standards.
