The information below:
- Demonstrates the aggregate amount of shared savings or shared losses earned by Pioneer Valley Accountable Care (PVAC) for each performance year.
- Provides statistics on how PVAC performs on Next Generation Model ACO quality measures for 2016 through 2020. The quality scores for 2021 will be posted in 2022
- Provides details of an arrangement PVAC has with physicians serving aligned beneficiaries in skilled nursing facilities and regarding PVAC's home visit program (see Public Disclosure Document).
